Laser beams ionize particles in the air . Ionized particles are conductive. As they are lined up, they form a path.
In order to force a lightning strike you need to bring the charged particles closer together, increase conductivity of the air or increase the amount of charged particles. If a bullet can do any of this, it may cause a lightning strike

Final costs depend on the healthcare provider. They can add an additional fee. On top comes a fee for special care for long term disability. That is technically a different system but is paid the same way. It may ne noteworthy that these fees are paid before income tax is calculated.
You technically can fall out off the system, bit it rarely happens. Not sure about the details though.
