Cooking to death has probably happened more, but pressing to death without heat has to be second or third in the list of food-preparation-related ways to kill people. “Chopping-up” has to be first, though.

I don’t know about industrial accidents with a cold press, but crushing(juicing) people to death has been a thing for millenias.
